Sermorelin

Sermorelin Acetate

Peptide

Sermorelin is a synthetic analog of growth hormone-releasing hormone (GHRH), consisting of the first 29 amino acids of endogenous GHRH, which represent the biologically active fragment. It is primarily used clinically to stimulate the pituitary gland to produce and secrete endogenous growth hormone, making it a popular therapeutic option for adult growth hormone deficiency, age-related GH decline, and off-label anti-aging and body composition purposes.

Mechanism of Action

Sermorelin binds to GHRH receptors (GHRHR) on somatotroph cells in the anterior pituitary gland, stimulating the synthesis and pulsatile release of endogenous growth hormone in a physiologically regulated manner. The released GH subsequently triggers hepatic production of insulin-like growth factor 1 (IGF-1), which mediates the downstream anabolic, lipolytic, and tissue-repair effects. Because sermorelin works through the hypothalamic-pituitary axis rather than bypassing it, natural negative feedback mechanisms remain intact, reducing the risk of GH excess compared to exogenous GH administration.

Dosing Protocols

injection200-500mcg- Once daily

30-60 minutes before bed on an empty stomach, coinciding with the natural nocturnal GH pulse

Cycle: 5 days on, 2 days off weekly; reassess every 3-6 months with IGF-1 labs

Administered as subcutaneous injection, typically into the abdomen. Avoid eating 1-2 hours before and after injection as elevated insulin blunts GH release. Starting dose of 200mcg is common; titrate upward based on IGF-1 response and tolerability. Some protocols use 100-300mcg twice daily (morning and before bed) for enhanced effect.

Safety & Side Effects

Sermorelin has a well-established safety profile compared to exogenous recombinant human GH, as it preserves physiological feedback regulation and is unlikely to cause GH excess under normal use; however, it is a prescription medication in the United States and many other countries, and should only be used under medical supervision with periodic monitoring of IGF-1 levels, fasting glucose, and thyroid function. Use is contraindicated in active malignancy, and caution is warranted in patients with hypothyroidism, diabetes, or conditions sensitive to fluid retention.

Possible Side Effects

  • !Injection site reactions including redness, swelling, or pain
  • !Facial flushing shortly after administration
  • !Headache, typically transient and dose-dependent
  • !Dizziness or lightheadedness, especially at higher doses
  • !Water retention and mild peripheral edema
  • !Nausea, particularly when food is consumed close to injection time
  • !Somnolence or fatigue following injection due to GH release
  • !Rare hypersensitivity or allergic reaction to peptide or excipients

Interactions

  • -Glucocorticoids (e.g., prednisone) blunt pituitary GH response and may significantly reduce sermorelin efficacy
  • -Insulin and other hypoglycemic agents: sermorelin-stimulated GH can cause transient insulin resistance, potentially requiring dose adjustments in diabetic patients
  • -Thyroid hormone replacement: hypothyroidism attenuates GH response; adequate thyroid status is necessary for optimal efficacy
  • -Somatostatin analogs (e.g., octreotide) directly antagonize GHRH signaling and will inhibit sermorelin's mechanism of action
  • -Exogenous testosterone or estrogen therapy may modify IGF-1 response and should be disclosed to prescribing physician when co-administering

Cost & Where to Buy

$80-$300
per month

Cost varies significantly based on source: compounding pharmacy prescriptions typically range $150-300/month including consultation, while research-grade peptide vendors (where legal) may offer lower prices of $80-150/month. Requires reconstitution with bacteriostatic water. Consultation and lab monitoring add additional costs not reflected here.
