SAMe

S-Adenosyl-L-Methionine

Supplement

SAMe (S-Adenosyl-L-Methionine) is a naturally occurring compound synthesized in the body from methionine and ATP, widely used as a supplement for depression, liver health, and osteoarthritis. It serves as the primary methyl donor in over 100 biochemical reactions and is one of the most thoroughly researched nutraceuticals for mood and joint support.

Mechanism of Action

SAMe donates methyl groups in transmethylation reactions critical for the synthesis of neurotransmitters including serotonin, dopamine, and norepinephrine, as well as phosphatidylcholine for neuronal membrane integrity. It also supports transsulfuration pathways producing glutathione, the body's primary antioxidant, which underlies its hepatoprotective effects. In joints, SAMe stimulates proteoglycan synthesis by chondrocytes and exerts anti-inflammatory effects comparable to NSAIDs in some trials.

Dosing Protocols

oral400-1600mg- Twice to three times daily

On empty stomach, 30 minutes before meals

Start at 200-400mg/day and titrate up over 2-4 weeks to minimize GI side effects. For depression, 800-1600mg/day is typically required; for osteoarthritis, 600-1200mg/day is commonly used. Use enteric-coated formulations for best bioavailability and stability.

Safety & Side Effects

SAMe has a generally favorable safety profile in healthy adults and has been used clinically in Europe for decades, but it carries a meaningful risk of triggering hypomania or mania in individuals with bipolar disorder and should be avoided or used only under close psychiatric supervision in that population. Co-supplementation with B12, B6, and folate is recommended to prevent homocysteine accumulation from chronic use.

Possible Side Effects

  • !Nausea and gastrointestinal discomfort, especially when starting or at high doses
  • !Loose stools or diarrhea, particularly above 800mg/day
  • !Headache during initial dose titration
  • !Insomnia or sleep disturbances if taken in the evening
  • !Anxiety or restlessness, particularly in sensitive individuals
  • !Hypomania or mania induction in individuals with bipolar disorder
  • !Dry mouth
  • !Mild elevations in homocysteine if B-vitamin cofactors are insufficient

Interactions

  • -Contraindicated or use with extreme caution with MAOIs - risk of serotonin syndrome
  • -May enhance serotonergic effects of SSRIs, SNRIs, and tricyclic antidepressants - monitor for serotonin syndrome symptoms
  • -May potentiate the effects of levodopa in Parkinson's patients while potentially reducing its efficacy over time
  • -Alcohol and hepatotoxic drugs - SAMe may be protective but does not eliminate liver risk; avoid combining with heavy alcohol use
  • -B-vitamin deficiency (B12, folate, B6) impairs SAMe metabolism and increases homocysteine - ensure adequate cofactor status

Cost & Where to Buy

$25-$90
per month

Cost varies significantly by dose and brand; higher therapeutic doses (800-1600mg/day) substantially increase monthly cost. Enteric-coated, pharmaceutical-grade formulations from reputable brands cost more but offer better stability and absorption. SAMe is sold OTC in the US but is prescription-only in several European countries.
