Psyllium husk is a soluble dietary fiber derived from the seeds of Plantago ovata, primarily used as a bulk-forming laxative and cholesterol-lowering agent. It is one of the most well-studied dietary fibers, with FDA-approved health claims for reducing the risk of heart disease when combined with a low-fat diet. It is widely used for managing constipation, diarrhea, irritable bowel syndrome (IBS), and metabolic conditions including hypercholesterolemia and type 2 diabetes.

Mechanism of Action

Psyllium husk forms a viscous gel in the gastrointestinal tract upon contact with water, slowing gastric emptying and intestinal transit, which reduces postprandial glucose absorption and promotes satiety. The gel-forming mucilage binds bile acids in the intestinal lumen, interrupting enterohepatic circulation and forcing the liver to convert additional cholesterol into bile acids, thereby lowering LDL cholesterol. Fermentation of psyllium by colonic microbiota also produces short-chain fatty acids (SCFAs) such as butyrate, which support colonocyte health and modulate gut microbiome composition.

Dosing Protocols

Start with 5g once daily and gradually increase over 1-2 weeks to minimize bloating and gas. For cholesterol reduction, clinical trials typically use 10-15g/day in divided doses. Always take with adequate water to prevent esophageal or intestinal obstruction. Do not take dry.

Psyllium husk has an excellent long-term safety profile and is considered safe for daily use in most adults, including during pregnancy when used at recommended doses. Caution is warranted in individuals with dysphagia, esophageal narrowing, or pre-existing bowel obstruction, and it must always be taken with ample water; rare but serious cases of esophageal and intestinal obstruction have been reported.
