Nattokinase is a serine protease enzyme extracted from natto, a traditional Japanese fermented soybean food, produced by Bacillus subtilis var. natto during fermentation. It is primarily used as a cardiovascular supplement to support healthy blood flow, reduce thrombotic risk, and promote fibrinolysis by breaking down fibrin clots. Research also supports its use for blood pressure management and as a potential alternative or adjunct to conventional anticoagulant therapy in low-risk individuals.

Mechanism of Action

Nattokinase exerts its primary effects by directly cleaving fibrin—the protein scaffold of blood clots—mimicking and enhancing the body's endogenous fibrinolytic system. It degrades plasminogen activator inhibitor-1 (PAI-1), thereby increasing circulating tissue plasminogen activator (tPA) activity, which amplifies plasmin-mediated clot dissolution. Additionally, nattokinase has demonstrated modest ACE-inhibitory activity, contributing to blood pressure reduction by decreasing angiotensin II-mediated vasoconstriction.

Dosing Protocols

Dosing is standardized in Fibrinolytic Units (FU); 2,000 FU once daily is the most studied dose for cardiovascular support. Nattokinase has a generally favorable safety profile in healthy adults at standard doses of 2,000 FU daily, with several human clinical trials demonstrating tolerability over months of use. However, its potent fibrinolytic activity poses a meaningful bleeding risk—particularly in individuals on anticoagulants, antiplatelet drugs, or those with hemorrhagic disorders—and it should be discontinued at least 2 weeks before any surgical procedure.
