Methylene Blue

Methylene Blue (Low-Dose)

Supplement

Methylene Blue is a phenothiazine dye with a long medical history, now gaining attention as a nootropic and mitochondrial support agent at low doses (0.5–4 mg/kg or roughly 35–280mg for a 70kg adult, though cognitive protocols typically use far lower amounts of 0.5–2mg/kg). At low doses it acts as an alternative electron carrier in the mitochondrial electron transport chain, and is used off-label to support cognitive function, memory, and neuroprotection. It has FDA-approved uses at higher doses for methemoglobinemia treatment but is explored at sub-therapeutic doses for longevity, mood, and mental clarity.

Mechanism of Action

Methylene Blue functions as a redox cycling agent that donates electrons directly to the mitochondrial electron transport chain at complexes I and IV, bypassing dysfunctional segments and increasing ATP production efficiency. At low doses it acts as a monoamine oxidase inhibitor (MAOI) and increases cytochrome c oxidase activity, enhancing cerebral oxygen consumption. It also reduces reactive oxygen species (ROS) at low concentrations through a hormetic mechanism, and inhibits nitric oxide synthase and soluble guanylate cyclase, contributing to its vasoconstrictive and neuroprotective effects.

Dosing Protocols

oral0.5–2 mg/kg body weight (approximately 35–140mg for a 70kg adult); nootropic sweet spot often cited at 0.5–1 mg/kg (~35–70mg)- Once daily or every other day

Morning, with a small meal to reduce gastrointestinal discomfort; avoid afternoon dosing due to potential sleep disruption

Cycle: 5 days on, 2 days off; or 4 weeks on, 1–2 weeks off to prevent tolerance and assess baseline

Use pharmaceutical-grade USP methylene blue solution (1% = 10mg/mL) to avoid industrial-grade contaminants. Urine and stool will turn blue-green, which is harmless. Start at the lower end of the range and titrate up. Avoid doses above 2 mg/kg without medical supervision.

Safety & Side Effects

Low-dose methylene blue (0.5–2 mg/kg) has a generally favorable safety profile in healthy individuals without G6PD deficiency, and pharmaceutical-grade preparations are well-tolerated for short-to-medium-term use; however, it carries a clinically significant risk of serotonin syndrome when combined with SSRIs, SNRIs, MAOIs, or other serotonergic agents, and this interaction has resulted in serious adverse events including deaths. It is an FDA-approved drug at higher doses and is unscheduled as a supplement in the US, but pharmaceutical-grade sourcing is essential as industrial-grade methylene blue contains heavy metal contaminants.

Possible Side Effects

  • !Blue-green discoloration of urine and stool (universal, dose-dependent, harmless)
  • !Temporary bluish tint to skin or mucous membranes at moderate-to-high doses
  • !Nausea and gastrointestinal discomfort, especially on an empty stomach
  • !Headache, particularly during early use or at higher doses
  • !Anxiety or agitation, more common at doses above 2 mg/kg
  • !Sleep disruption or insomnia if taken in the afternoon or evening
  • !Serotonin syndrome risk when combined with serotonergic drugs (serious, dose-dependent)
  • !Hemolytic anemia in individuals with G6PD deficiency (contraindicated in this population)

Interactions

  • -Contraindicated with SSRIs, SNRIs, TCAs, MAOIs, and other serotonergic drugs — risk of potentially fatal serotonin syndrome
  • -May potentiate effects of stimulants (amphetamines, caffeine) due to monoamine activity — use with caution
  • -Can increase blood pressure — use caution with antihypertensive medications as effects may be unpredictable
  • -Concurrent use with nitric oxide donors or PDE5 inhibitors (e.g., sildenafil) may blunt their vasodilatory effects via guanylate cyclase inhibition
  • -Contraindicated in G6PD deficiency — may cause acute hemolytic anemia

Cost & Where to Buy

$10-$40
per month

Pharmaceutical-grade USP 1% methylene blue solution (e.g., from Methylene Blue USA or equivalent) typically costs $15–$35 for a 30–60mL bottle, which covers several weeks of low-dose use. Industrial or lab-grade methylene blue should never be used. Some compounding pharmacies also offer capsules at slightly higher cost.
