Aged Garlic Extract

Aged Garlic Extract (Allicin)

Supplement

Aged Garlic Extract (AGE) is a standardized supplement produced by prolonged cold aging of raw garlic, which converts harsh sulfur compounds into stable, bioavailable organosulfur compounds including S-allylcysteine (SAC) and S-allylmercaptocysteine (SAMC). It is primarily used for cardiovascular support, immune modulation, and antioxidant protection, with the most robust evidence supporting modest reductions in blood pressure and LDL cholesterol. Unlike raw garlic or allicin supplements, AGE is odorless and well-tolerated, making it suitable for long-term daily use.

Mechanism of Action

The primary bioactive compound in AGE, S-allylcysteine, exerts cardiovascular effects by inhibiting HMG-CoA reductase activity, reducing oxidative stress via upregulation of Nrf2-dependent antioxidant enzymes (including glutathione-S-transferase and superoxide dismutase), and modulating nitric oxide synthase to promote endothelial vasodilation. AGE also demonstrates anti-inflammatory activity by suppressing NF-κB signaling and reducing pro-inflammatory cytokines such as TNF-α and IL-6. Additionally, organosulfur compounds inhibit platelet aggregation by interfering with thromboxane A2 synthesis, contributing to its anticoagulant properties.

Dosing Protocols

oral600-1200mg- Once daily

With meals

Most clinical trials use 600-1200mg of standardized AGE daily. Products should be standardized to contain at least 0.1% S-allylcysteine per dose. Doses up to 2400mg/day have been used in cardiovascular studies without significant adverse effects. Kyolic-brand AGE is the most extensively studied commercial product.

Safety & Side Effects

Aged Garlic Extract has an excellent safety profile and is well-tolerated in most populations, including long-term daily use at recommended doses; however, its antiplatelet and mild anticoagulant properties require caution in individuals taking blood thinners, those with bleeding disorders, or those scheduled for surgery. It is not a prescription substance and is widely available OTC, but individuals on antihypertensive or lipid-lowering medications should consult a healthcare provider due to potential additive effects.

Possible Side Effects

  • !Mild gastrointestinal discomfort including bloating or nausea, particularly at higher doses
  • !Loose stools or diarrhea at doses above 1200mg/day
  • !Mild residual garlic breath or body odor (less pronounced than raw garlic)
  • !Headache reported in a small subset of users during initial use
  • !Increased bleeding time or bruising due to antiplatelet effects
  • !Rare allergic reactions including skin rash in individuals with garlic hypersensitivity

Interactions

  • -May enhance anticoagulant and antiplatelet effects of warfarin, aspirin, clopidogrel, or NSAIDs — monitor INR and bleeding risk
  • -Additive blood pressure-lowering effect when combined with antihypertensive medications — may require dose adjustment
  • -Can potentiate lipid-lowering effects of statins, which may be beneficial but warrants monitoring for adverse effects
  • -May reduce efficacy of saquinavir (HIV protease inhibitor) — avoid concurrent use
  • -Additive antiplatelet effects with fish oil, nattokinase, or high-dose vitamin E supplements — use caution in combination

Cost & Where to Buy

$12-$35
per month

Cost varies by brand, standardization, and dose per capsule. Kyolic AGE products are among the most studied but priced at a premium (~$25-35/month at 600-1200mg/day). Generic or store-brand AGE products are available for as low as $12/month. Standardization to S-allylcysteine content is a key quality marker.
